METABOLIC HEALTH—

It's Not Your Fault.
It May Be Your Insulin.

If you're South Asian and your weight looks normal paper, your cardiometabolic risk may still not be. Hidden visceral fat, low lean muscle, and insulin resistance can rnake fat loss harder and increase the risk of prediabetes, fatty liver, and heart disease.

Your BMI is lying to you

BMI measures weight, not where fat sits. Visceral fat , the kind wrapped around your liver and pancreas drives insulin resistance, raises inflammation, and lowers testosterone. Subcutaneous fat, the kind you can pinch, does far less damage.

South Asian men carry more visceral fat at every BMI than men of European descent. That's why standard cutoffs miss it.

Does this sound like you?

  • ✓ Belly weight that won't budge
  • ✓ Normal weight, but soft through the middle
  • ✓ Thin arms or low muscle tone
  • ✓ Low energy or stalled progress

What insulin resistance can do

  • ✓ Make belly fat harder to lose
  • ✓ Slow metabolic efficiency
  • ✓ Increase cravings and energy dips
  • ✓ Raise cardiometabolic risk

How we help

  • ✓ GLP therapy
  • ✓ Personalized plans
  • ✓ Peptide therapy
  • ✓ Nutrition and metabolic support
  • ✓ Body-composition-focused care

Where testosterone comes in

Fat tissue converts testosterone to estradiol. More visceral fat means more conversion and less available testosterone which makes it harder to build muscle and easier to store fat around the middle. The cycle tightens over years, quietly.

Different thresholds, different risk

Measure Standard cutoff South Asian cutoff
Overweight (BMI) 25.0 23.0
Obesity (BMI) 30.0 27.5
Waist circumference (men) 40 in / 102 cm 35 in / 90 cm

A South Asian man at BMI 24 with a 36–inch waist is "normal weight" by standard criteria — and already past the threshold that applies to him.

What we actually measure

A standard physical won't find this. We test the markers that reveal visceral fat and insulin resistance before they become a diagnosis.

Hormonal

Total and free testosterone, SHBG, estradiol, LH and FSH, thyroid function

Metabolic

HbA1c, fasting glucose and insulin. HOMA-IR, liver enzymes, triglyceride-to-HDL ratio

Cardiovascular

Full lipid panel, APOB, lipoprotein(a), hs-CRP, blood pressure

Body composition

Waist circumference, waist—to-height ratio, body fat percentage and distribution

How we treat it

Visceral fat responds. It's often the first fat to move — but only when the plan targets the actual driver rather than the number on the scale.

Hormone optimization

Where labs confirm deficiency, restoring testosterone to mid-normal range improves lean mass and reduces visceral adiposity interrupting the cycle at its source.

GLP-I therapy

Semaglutide and tirzepatide reduce visceral fat and improve insulin sensitivity. Paired with adequate protein and resistance training to protect muscle.

Targeted peptides

Tesamorelin is FDA-approved specifically for the reduction of visceral adipose tissue, and is considered where clinically appropriate.

Metabolic foundation

Nutrition built around how you actually eat, resistance training, sleep, and ongoing lab monitoring not a generic plan.
